My float job failed on a 90 degree day in Dallas last August

Finished a 4000 sq ft driveway and the whole surface turned to dust in a week. Turns out I mixed the cure rate wrong and the finishers were working it too wet in the heat. Has anyone else had a big job go bad from a basic mix mistake?
